LAwell Medical Plan Survey: We Want to Hear from You
Your LAwell Civilian Employee Benefits Program (LAwell) is exploring possible changes to its medical plan offerings, and we want your input.
LAwell currently offers a Preferred Provider Option (PPO) medical plan through Anthem Blue Cross and multiple Health Maintenance Option (HMO) medical plans through Anthem Blue Cross and Kaiser Permanente. We'd like to know about your experiences with these plans and what changes you'd like to see.

Focus Groups for LAwell Medical Plans
A Focus Group is a meeting where you'll be asked open-ended questions about your experiences. An impartial third-party representative will collect your feedback.
To participate in a Focus Group for LAwell Medical Plans, please register** in one of the following ways

Everyone in a Focus Group will be entered into a raffle for a chance to win a gift card. Winners can refuse the gift card or sign an acknowledgment that its value will be reported as taxable income on their W2 tax form.
**Please Note:
· In-person space is limited, and registration does not guarantee attendance. Look for a confirmation email.
· Attendance is voluntary, and attendance during work hours should be arranged with your supervisor.
· In-person events do not provide parking, food, or beverages.
Survey for LAwell Medical Plans
In early December, LAwell will distribute a survey via a separate Citywide email, but employees will also have an option to request a paper copy. Employees who complete the survey can "opt in" to be entered into a raffle for a chance to win a gift card.
